Position Overview
The Home Stretch Transition Coach plays a crucial role in providing one-on-one professional support and case coordination for young individuals transitioning from Out-of-Home-Care until the age of 21. This role is vital in guiding these young people towards independence and stable living arrangements.
This position involves a commitment of 38 hours per week, with flexible start and finish times agreed upon with the line manager.
Key Responsibilities
The successful candidate will be responsible for:
- Conducting interviews and assessments to understand the support needs of young people referred to Home Stretch WA.
- Offering regular mentoring and coaching to develop skills, connections, and self-confidence in young individuals.
- Supporting the development of Staying On Agreements to ensure stable living arrangements with foster or family carers.
- Assisting in finding and maintaining safe housing using available resources, including the Home Stretch WA Housing Allowance.
- Delivering culturally safe, person-centred support in line with the Home Stretch WA Model.
- Advocating for young people while working collaboratively with stakeholders such as foster carers, communities, and the Department of Communities.
- Maintaining accurate service records and responding to queries professionally.
- Participating in team meetings and professional development activities.
Candidate Profile
We are seeking a passionate individual with the following qualifications and attributes:
- A current C-class driving licence.
- Willingness to obtain a recent Police Clearance, Working with Children Check, and Senior First Aid Certificate.
- Certificate IV or equivalent in Community or Human Services (desirable).
- Strong understanding of social, cultural, and economic issues affecting young people.
- Excellent customer service and interpersonal skills.
- Effective communication skills adaptable to various audiences.
- Attention to detail and strong organisational abilities.
- Commitment to maintaining a safe workplace and reporting any risks or hazards.
- Proficiency in Microsoft Office and other computer systems.
